How to Apply

  1. Obtain Required Education and Clinical Hours ▾
    Complete a Master's degree in Speech-Language Pathology from an accredited institution and finish the required supervised clinical practicum hours.
  2. Pass the Praxis Examination ▾
    Register for and pass the Praxis Subject Assessment in Speech-Language Pathology (test code 5331). Ensure scores are sent to the Arizona Department of Health Services.

  3. Apply for a Temporary License (Clinical Fellowship) ▾
    If you are a new graduate, apply for a temporary license to complete your Clinical Fellowship Year (CFY). This license is valid for one year.
