How to Apply

    Schedule and complete a fingerprint-based background check with an Illinois-licensed live scan vendor within 60 days of application submission.
  4. Submit Official Transcripts
    Request your university to send official transcripts directly to IDFPR verifying your doctoral degree (AuD).
  5. Send Praxis Scores
    Request ETS to send your official Praxis Audiology (test code 5343) scores directly to the Illinois board.

Steps

  1. Verify Out-of-State License
    Submit Supporting Document CT to the state board where you hold your original and current licenses to be sent to IDFPR.
  2. Submit ASHA CCC-A
    Provide a copy of your current ASHA Certificate of Clinical Competence in Audiology, which may waive certain experience verifications.

How many CE hours are required for renewal?
