What You'll Need

How to Apply

  1. Create an ILogin Account ▾
    Register for a State of Illinois ILogin account to access the CORE licensing portal.
  2. Submit Online Application ▾
    Complete the Audiologist application within the CORE portal, selecting the appropriate licensure method (Examination or Endorsement).
  3. Complete Fingerprinting ▾
    Schedule and complete a fingerprint-based background check with an Illinois-licensed live scan vendor within 60 days of application submission.
  4. Submit Official Transcripts ▾
    Request your university to send official transcripts directly to IDFPR verifying your doctoral degree (AuD).
  5. Send Praxis Scores ▾
    Request ETS to send your official Praxis Audiology (test code 5343) scores directly to the Illinois board.

Audiologists licensed in another state can often apply via endorsement by providing license verification and current certification.

Steps

  1. Verify Out-of-State License ▾
    Submit Supporting Document CT to the state board where you hold your original and current licenses to be sent to IDFPR.
  2. Submit ASHA CCC-A ▾
    Provide a copy of your current ASHA Certificate of Clinical Competence in Audiology, which may waive certain experience verifications.
  3. Pay Endorsement Fee ▾
    The fee for licensure by endorsement is $145 ($100 application + $45 consumer protection fee).

Missing Consumer Protection Fee ▾
Failure to include the mandatory $45 Hearing Instrument Consumer Protection fee with the base application fee will stall processing.
Fingerprint Expiration ▾
Fingerprints must be taken within 60 days of the application date; if taken too early or late, they must be redone.
Incomplete CT Forms ▾
For endorsement, boards often fail to send the CT form directly to IDFPR or omit disciplinary history.

Questions

How many CE hours are required for renewal? ▾
Audiologists must complete 20 hours of continuing education every two years, including 2 hours of ethics, 1 hour of sexual harassment training, and 1 hour of implicit bias training.
When do Illinois Audiologist licenses expire? ▾
All licenses expire on October 31 of odd-numbered years.
Can I practice while my application is pending? ▾
Out-of-state license holders with ASHA CCC-A may practice for up to 90 days from the date of application or until the application is decided, whichever is sooner.
Is a doctoral degree required? ▾
Yes, for any applicant whose degree was conferred on or after January 1, 2008, a doctoral degree in audiology is required.
