How to Apply

Enroll in an Approved Radiography Program ▾Enroll in a program accredited by the Commission on Dental Accreditation (CODA) or a program approved by the Indiana Department of Health (IDOH).

Apply for a Provisional Permit ▾If you are not in a CODA-accredited program, you must apply for a Provisional Permit to perform clinical training. This requires a signature from your program director.
