How to Apply

Create MyLicense One Account ▾Register for an account on the Indiana MyLicense One portal using Access Indiana for secure login.
Register/Login -
Submit Online Application ▾Complete the Occupational Therapist application and pay the $100 non-refundable application fee via credit or debit card.
Apply Online -
Request Official Transcripts ▾Contact your educational institution to send official transcripts directly to the Indiana Occupational Therapy Committee showing your degree conferred.

Transfer NBCOT Scores ▾Request that NBCOT send your official score report directly to the Indiana Occupational Therapy Committee.

Complete Criminal Background Check ▾After submitting your application, you will receive an email with a service code. Use this code to schedule your fingerprinting appointment with IdentoGO.

Occupational therapists licensed in another state can often apply via endorsement by providing license verification and current NBCOT certification.

How many continuing education hours are required for renewal?
▾
Occupational Therapists must complete 18 hours of continuing competency every two years, with at least 9 hours in Category I courses.
When does my Indiana OT license expire?

Licenses expire on December 31 of even-numbered years.
Is there a Jurisprudence Exam required for Indiana OT licensure?

No, Indiana does not currently require a separate Jurisprudence Exam for OT or OTA applicants.
Can I practice while my application is pending?

You may apply for a temporary permit for an additional $50 fee, which allows practice under the direct supervision of an Indiana-licensed OT while awaiting exam results or full licensure.
What is the total cost for initial licensure?

The application fee is $100. If a temporary permit is requested, the total is $150. Background check fees are paid separately to the vendor (approx. $38).
