How to Apply

  1. Submit Online Application
    Create an account on the MyLicense One portal and submit the initial application for Speech-Language Pathology licensure.

  2. Pay Application Fee
    Pay the non-refundable $150.00 application fee via credit or debit card through the online portal.
  3. Complete Criminal Background Check
    Wait for the 'Indiana Licensure Application Received' email containing your specific Service Code. Use this code to schedule a fingerprinting appointment with IdentoGO. Do not fingerprint before receiving this code.

  4. Submit Required Credentials
    Request official transcripts, Praxis scores (code 0089), and Clinical Fellowship verification to be sent directly to the Board. If you hold ASHA CCC-SLP, you may submit a verification letter from ASHA instead.

  5. Pass Jurisprudence Examination
    Once your application is reviewed and approved, the Board will send you the Jurisprudence Exam. You must pass with a score of at least 75%.
  6. License Issuance
    After all documents are received and the Jurisprudence Exam is passed, the Board will issue your license.

Questions

How many continuing education hours are required for renewal?
Licensed SLPs must complete 36 hours of continuing education every two years. No more than 6 hours may be earned through self-study.
When does my Indiana SLP license expire?
Licenses expire on December 31st of every odd-numbered year.
Can I practice while my application is pending?
No. You must have an active Indiana license issued by the Board to practice as an SLP in Indiana, including in school districts.
Does Indiana accept ASHA CCCs in lieu of other documents?
Yes, a current ASHA CCC-SLP can substitute for official transcripts, clinical experience documentation, and Praxis scores, but you must still pass the Jurisprudence Exam.
