How to Apply

  1. Order Fingerprint Card ▾
    Request an official FD-258 fingerprint card from the Board of Pharmacy website. The Board will mail the card to you within one business day.
  2. Complete Fingerprinting ▾
    Take the FD-258 card to a local law enforcement agency or an approved fingerprinting vendor to have your prints taken. Ensure the card is filled out with the Board's ORI (KS920152Z).
  3. Complete Application Forms ▾
    Download and type the Pharmacy Technician Registration Application (Form LA-14) and the KBI/FBI Criminal Background Check Waiver (Form S-100). Handwritten forms will be returned without processing.
  4. Prepare Payment ▾
    Prepare a check or money order for $124 ($77 application fee + $47 background check fee) payable to the Kansas State Board of Pharmacy.
  5. Mail Application Packet ▾
    Mail the completed Form LA-14, Form S-100, fingerprint card, copy of photo ID, and fees to the Board office at 800 SW Jackson, Suite 1414, Topeka, KS 66612.
