Create LaMED DashBoard Account ▾Register for an account on the official LSBME online portal to begin the initial application process.

Submit Online Application and Pay Fees ▾Complete the online application for Occupational Therapist licensure and pay the required $200 fee (includes $150 licensure fee and $50 temporary license fee) via credit/debit card.

Submit Notarized Forms ▾Download, print, and notarize the Oath or Affirmation and Third Party Authorization forms. Mail the original notarized documents to the LSBME office.
