What You'll Need

• Official Transcripts — Must show completion of a master's or doctoral degree in audiology and be sent directly from the educational institution.
• Praxis Score Report — Official scores from the Praxis Series II Audiology exam (Test Code 5343) sent via ETS using code 7430. ETS Praxis Site
• Clinical Audiology Work Experience Form — Required for applicants without an Au.D. to verify 9 months (1,080 hours) of supervised experience.
• Human Trafficking Training Proof — Attestation or certificate of completion for a one-time training identifying victims of human trafficking.
• Implicit Bias Training Proof — Proof of 2 hours of implicit bias training completed within the 5 years preceding the application.

How to Apply

1. Create a MiPLUS Account ▾
   Register for an account on the Michigan Professional Licensing User System (MiPLUS) to begin the application process.

2. Submit Online Application ▾
   Complete the Audiologist by Examination application and pay the required fee of $463.50 via credit or debit card.

3. Complete Criminal Background Check ▾
   After submitting your application, you will receive an email with instructions and a code to schedule your fingerprinting appointment through IdentoGO.

4. Submit Official Transcripts ▾
   Request your university to send official transcripts directly to the Michigan Bureau of Professional Licensing (BPL) via email to bpldata@michigan.gov.
5. Report Praxis Scores ▾
   Have your Praxis Series II Examination in Audiology scores sent directly from ETS to the Michigan Board using recipient code 7430.

6. Verify Clinical Experience ▾
   If you do not hold an Au.D., have your supervisor submit the Clinical Audiology Work Experience form verifying 1,080 clock hours directly to the board.
