Complete Educational Requirements ▾Obtain a Master's or Doctoral degree in Audiology from a program registered by the Department or accredited by ASHA's Council on Academic Accreditation.

Pass the Praxis Examination ▾Register for and pass the Praxis II Audiology exam (test code 5343) with a minimum score of 162. Ensure scores are sent directly to the New York State Education Department.

Complete Supervised Experience ▾Complete nine months of full-time supervised experience (for Master's degree holders) or 1,820 hours of supervised clinical experience (for Doctoral degree holders).

Submit Application Form 1 ▾Complete the online Application for Licensure (Form 1) and pay the $294 fee for licensure and first registration.

Submit Supporting Forms ▾Have your university submit Form 2 (Certification of Professional Education) and your supervisor submit Form 4A (Identification of Supervisor and Setting) directly to the Office of the Professions.
