How to Apply

  1. Submit Online Application (Form 1)
    Complete the online Form 1 application and pay the required $294 fee for licensure and first registration.
  2. Verify Professional Education (Form 2)
    Request your physical therapy school to complete Form 2 (Certification of Professional Education) and send it directly to the NYSED Office of the Professions.

  3. Register for the NPTE
    Create an account with the Federation of State Boards of Physical Therapy (FSBPT) and register for the National Physical Therapy Examination (NPTE).

  4. Receive Authorization to Test (ATT)
    Once NYSED verifies your eligibility, you will receive an ATT letter from FSBPT via email.
  5. Schedule and Pass the Exam
    Schedule your exam date through Prometric using the instructions in your ATT letter. Pass the NPTE with a score of 600 or higher.
  6. Final Review and Licensure
    NYSED will review your passing scores and all submitted documents. Once approved, your license number will be issued and appear in the online verification system.
