How to Apply

  1. Verify Eligibility ▾
    Ensure you have a doctoral degree (AuD) from an accredited institution (if graduated after Jan 1, 2008) and have completed 1,800 clock hours of supervised professional experience.

  2. Pass the Praxis Exam ▾
    Register for and pass the Praxis Audiology exam (test code 5343) administered by ETS. Ensure scores are sent to the Board.

    Visit the Board's homepage and create an account in the licensee portal to begin the application process.

    Must hold an active, full, and unrestricted license in another state.
  2. Submit Reciprocal Application ▾
    Complete the online Reciprocal Application form through the licensee portal.

What are the continuing education (CE) requirements? ▾
Each licensee must complete 30 hours of continuing professional education every three years. You may elect to sync your cycle with your ASHA cycle.
Can I practice before my license is issued? ▾
No. You are in violation of the law if you practice before your license is issued, unless your practice is in an exempt area.
What are the total fees for initial licensure? ▾
The application fee is $30 (non-refundable) and the permanent license fee is $60. Online payments include a convenience charge.
Are there fee exemptions for military members? ▾
Licensees or spouses with Active-Duty Status are exempt from paying the renewal fee upon submission of official documentation.
