What You'll Need

  • Passport Photo — A photo taken against a plain white wall; no copies of driver's licenses or actual passports accepted.
  • Citizenship Affidavit — Notarized form verifying U.S. citizenship or qualified alien status.
  • Three Reference Forms — Must be on the official Board form and completed by individuals in the same professional area (Audiology).
  • Official Transcripts — Sent directly from the university showing completion of an AuD or equivalent degree.
  • Praxis Scores or ASHA Verification — Evidence of passing the national examination or a copy of current ASHA CCC-A card with verification letter.

How to Apply

    Register for a new account on the OBESPA Online Licensing portal using a personal email address.

    Fill out the application boxes and upload a passport-style photo taken against a plain white wall.

    Upload the Citizenship Affidavit and three reference forms completed by licensed Audiologists or ASHA CCC-A holders.

    Perform a national criminal background check through the approved vendor, Trak-1.

    Request a verification letter from the state where you currently hold a license to be sent to OBESPA.
  2. Provide Current State Rules ▾
    Submit a copy of the licensure rules and a copy of your current licensure card from your current state.

    Submit the online application, $85 fee, citizenship affidavit, 3 reference forms, and Trak-1 background check.

Slow Downs

Using a background check service other than Trak-1; the Board only accepts results directly from Trak-1.

Questions

How many CE hours are required for renewal? ▾
Audiologists must complete 20 clock hours of continuing education every two years (due by Dec 31 of even-numbered years).
Is there an ethics requirement for CE? ▾
Yes, at least 3 of the 20 required hours must be in the area of professional ethics.
Can I get a temporary license? ▾
A temporary license is issued once all checklist items are received, allowing practice until the next Board meeting.
How do I request an official license verification for another state? ▾
Submit the Licensee Verification Request Form along with a $5.00 fee (check or money order) to the Board office.
