Training Program Certificate — Proof of completion from an OSDH-approved Nurse Aide Training and Competency Evaluation Program.
Valid Government-Issued Photo ID — A current driver's license, state ID, or passport required for testing and application.
Social Security Card — Original Social Security card must be presented at the testing site.
Affidavit of Lawful Presence (ODH Form 301) — Required for all applicants to verify citizenship or legal immigration status.
How to Apply
Complete State-Approved Training
▾
Complete a minimum of 75 hours of training, including at least 16 hours of supervised clinical instruction, at an OSDH-approved program.
Apply for the Competency Exam
▾
Submit your application to Prometric after completing your training. You can apply online or by mail.
Receive Authorization to Test (ATT)
▾
Once your application is approved, Prometric will issue an ATT letter. You must have this to schedule your exam.
Schedule and Pass Exams
▾
Schedule your written (or oral) and clinical skills exams through Prometric. You must pass both parts to be certified.
Automatic Registry Placement
▾
After passing both exam components, Prometric reports your results to the OSDH, and your name is automatically added to the Nurse Aide Registry.

Complete Reciprocity Application

Fill out the Reciprocity Application (ODH Form 735) for the Oklahoma Nurse Aide Registry.
Submit Required Documentation

Include a copy of your current out-of-state certificate, photo ID, and the signed Affidavit of Lawful Presence.
