How to Apply

Submit Online Application ▾Create an account and complete the online application for Physical Therapist licensure via the Allied Professionals Registration portal.

Pay Application Fee ▾Pay the non-refundable application fee of $150 online using a credit card, debit card, or EFT.

Register for NPTE ▾Register for the National Physical Therapy Examination (NPTE) through the Federation of State Boards of Physical Therapy (FSBPT).

Submit Evidence of Status ▾Complete, notarize, and email the Evidence of Status form along with required citizenship/alien status documentation to the Board.

Complete Background Check ▾After submitting your application, you will receive an email with a service code to schedule your biometric criminal background check.

Submit Official Transcripts ▾Request that your educational institution send official transcripts directly to the Oklahoma Medical Board.

Board Review ▾The Physical Therapy Advisory Committee reviews applications and makes recommendations to the Board for final approval.
