How to Apply

  1. Complete Pain Management Module ▾
    Complete the free one-hour online module 'Changing the Conversation about Pain' provided by the Oregon Pain Commission. Save your certificate.

    Create an account in the OBPT Self-Service Portal and submit the initial application for licensure by examination or endorsement.

  3. Pay Application Fees ▾
    Pay the non-refundable application fee of $187 via the online portal using a credit or debit card.

    Contact your PT education program to have official transcripts sent directly to the Board via email or mail.
  5. Pass the OR-JAM ▾
    Register for and pass the Oregon Jurisprudence Assessment Module (OR-JAM) through FSBPT. A score of 80% is required.

    Schedule a fingerprinting appointment with Fieldprint using the code provided by the Board after your application is submitted.
