  3. Submit Official Transcripts
    Request official transcripts for your Master's degree to be sent directly from your university to the Board. E-transcripts are preferred and should be sent to speechaud.board@bspa.oregon.gov.
  4. Verify Professional Competence
    Request verification of your ASHA CCCs to be sent to the Board, or ensure your Praxis exam scores are sent using Board code #8699.
  5. Complete Fingerprinting
    Schedule a fingerprinting appointment with Fieldprint using the specific code provided to you during the online application process.

    Request a letter of good standing from every state or jurisdiction where you have ever held a professional license (including teaching licenses) to be sent directly to the Board.

When does my Oregon SLP license expire?
