What You'll Need

• Official Transcripts — Must show completion of a Doctoral Degree in Audiology from an accredited institution.
• ASHA Certification (CCC-A) — Primary documentation for clinical competence; if not held, Praxis scores and fellowship verification are required.
• Praxis Exam Scores — Required if the applicant does not hold ASHA certification. Praxis Scores
• Clinical Fellowship/Externship Verification — Required for applicants without ASHA certification; must be completed on the form provided in the application packet.
• Fingerprint Authorization Form — Emailed by DOPL after application submission; required for the background check appointment.

How to Apply

1. Submit Initial Application ▾
   Complete the Audiologist application online via the MyLicenseOne portal or submit the manual PDF form to DOPL.

2. Pay Application Fee ▾
   Pay the non-refundable $70 application processing fee. If applying for both SLP and Audiologist licenses simultaneously, only one fee is required.
3. Submit Educational Transcripts ▾
   Request official transcripts showing a Doctoral Degree in Audiology (AuD) to be sent directly to DOPL.
4. Provide Professional Certification or Exam Scores ▾
   Submit documentation of current ASHA CCC-A certification OR documentation of passing the Praxis exam and completion of a clinical fellowship/externship.
5. Receive Fingerprint Authorization ▾
   After submitting your application, wait 2-3 business days for DOPL to email you a fingerprint authorization form and a list of approved vendors.

6. Complete Background Check ▾
   Visit an authorized vendor with your authorization form and a government-issued ID to have your fingerprints electronically submitted.
