What You'll Need

• Official Transcripts — Must be sent directly from an ACPE-accredited pharmacy program to DOPL.
• NAPLEX and MPJE Scores — Passing scores must be on file with NABP and accessible to the Utah Board. NABP Exam Services
• Fingerprint Cards — Two physical fingerprint cards or electronic submission through an authorized vendor for background check. Fingerprint Info
• Verification of Internship Hours — Documentation of 1,740 hours for U.S. graduates or 1,440 hours for foreign graduates.
• FPGEC Certificate (Foreign Graduates Only) — Copy of the Foreign Pharmacy Graduate Examination Committee certificate if applicable.

How to Apply

1. Pass Required Examinations ▾
   Register for and pass both the NAPLEX and the Utah MPJE (Multistate Pharmacy Jurisprudence Examination) through NABP. You must pass these before submitting your application to DOPL.

2. Submit Online Application ▾
   Create an account on the MyLicenseOne portal and submit the Pharmacist application. Alternatively, you can mail a manual paper application to DOPL.

3. Pay Application Fees ▾
   Pay the non-refundable application processing fee of $142. If applying for a Controlled Substance license simultaneously, additional fees may apply.
4. Submit Official Transcripts ▾
   Request your ACPE-accredited pharmacy school to send official transcripts directly to DOPL. Transcripts must show the degree conferred and graduation date.
5. Complete Fingerprinting ▾
   Complete the fingerprint-based background check. After applying, you will receive an authorization form to take to an approved vendor.

6. Verify Internship Hours ▾
   Ensure verification of at least 1,740 internship hours is submitted to DOPL. This is typically handled by your pharmacy school or the state board where hours were earned.
