How to Apply

  1. Complete Education ▾
    Graduate from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  2. Register for the NPTE ▾
    Create an account with the Federation of State Boards of Physical Therapy (FSBPT) and register for the National Physical Therapy Examination (NPTE).

  3. Submit DOPL Application ▾
    Submit your application for licensure to the Utah Division of Professional Licensing (DOPL) either online via the eGov portal or by mailing a manual application.

    Submit fingerprint cards for a background check. You must pay the BCI ($20) and FBI ($12) fees. DOPL no longer provides in-house fingerprinting; use an approved vendor.

Requirements ▾
Physical Therapists licensed in another state can often apply via endorsement by providing license verification and current certification.

Steps

  1. Verify Current License ▾
    Ensure your current out-of-state license is active and in good standing. You must have held it for at least one year to qualify for endorsement.
  2. Submit Endorsement Application ▾
    Complete the Utah Physical Therapist application and select the 'Endorsement' pathway.
  3. Request License Verification ▾
    Request an official verification of licensure from every state where you have ever held a license.

Slow Downs

Missing Fingerprints ▾
The #1 reason for delay. Applications are often processed within 21 days, and if fingerprints are not received, the application may be denied.
Transcript Delays ▾
Transcripts must come directly from the school. Delays in registrar processing can stall the entire application.
Incomplete Law Exam ▾
Failure to submit the completed take-home Law and Rule exam with the application packet.

Questions

How many CE hours are required for renewal? ▾
Physical Therapists must complete 40 hours of continuing education every two years. Physical Therapist Assistants must complete 20 hours.
Is there a specific ethics requirement for CE? ▾
Yes, at least 2 hours of the required CE must be in the area of ethics or law.
When do Utah PT licenses expire? ▾
Licenses expire on May 31 of every odd-numbered year.
What are the application fees? ▾
The application fee is $70 for Physical Therapists and $60 for Physical Therapist Assistants. Background check fees are additional ($20 BCI + $12 FBI).
