What You'll Need

• Official Transcripts — Must be sent directly from the college or university to the Board office.
• Praxis Score Report — Official scores must be sent directly from ETS to the Wyoming Board.
• Supervision Agreement — Required for provisional license applicants to define the supervisory relationship during the CFY. Supervision Forms
• Clinical Fellowship Rating Form — Completed by the supervisor to verify successful completion of the CFY. CFY Rating Form
• Proof of Lawful Presence — Documentation such as a birth certificate or passport as required by federal law.
• Fingerprint Cards — Two completed fingerprint cards for the required background check.

How to Apply

1. Obtain Master's Degree ▾
   Complete a Master's degree in Speech-Language Pathology from a CAA-accredited program, including at least 375 clinical practicum hours.
2. Pass the Praxis Exam ▾
   Take and pass the National SLP Examination (Praxis) with a score of 162 or higher. Scores must be sent directly to the Board from ETS.

3. Apply for Provisional Licensure ▾
   If you have not completed your Clinical Fellowship Year (CFY), submit the Provisional Licensure application packet and a $200 fee to begin your supervised practice.

4. Complete Clinical Fellowship ▾
   Complete a 36-week (9-month) supervised clinical fellowship under a licensed SLP while holding a Wyoming provisional license.
5. Submit Fingerprints ▾
   Submit fingerprint cards for a criminal history record background check. This process typically takes 3-5 weeks.

6. Apply for Full Licensure ▾
   Upon completion of the CFY, submit the 'Licensure by Completing Provisional' form, the Clinical Fellowship Rating Form, and the $100 fee.
